What we do

We work together to provide more coordinated, person-centred support to people in a specific community or area. The aim is to help individual, especially those with complex or ongoing health and social care needs, live healthier, more independent lives closer to home.

As an INT we:

  • Share information and work together to support people more effectively.

  • Identify people who need extra support.

  • Organise events, workshops and sessions to reach people in their community.

  • Help people to navigate health, social, and community services more easily.

  • Work to keep people at home and out of hospital wherever safe and possible.

Our Priorities

Wallington Wellbeing Integrated Neighbourhood Team and Sutton Primary Care Networks are working with London Borough of Sutton and local Mental Health Services to provide NHS services that are easy to access and helpful. We want to make sure everyone gets the care they need.

Dementia

Nationally, dementia is a priority for the NHS and social care services. The UK government has launched several initiatives to improve dementia care, including the Dementia Strategy and the Dementia Discovery Fund, which invests in research for better treatments and care methods. Diagnosis and Support Early diagnosis is crucial. The NHS aims to increase diagnosis rates so that people can access support and treatment sooner. Memory clinics and dementia advisors are available across the country to help with this. Research and Innovation The UK is a leader in dementia research. Organisations like Alzheimer’s Research UK and the Alzheimer’s Society fund research to find new treatments and ultimately a cure. Community Support There are numerous support groups and charities, such as Dementia UK and Age UK, that provide resources, advice, and support for people living with dementia and their carers. Awareness and Training Efforts are being made to increase public awareness and understanding of dementia. Training programs for healthcare professionals and carers are also being expanded to improve the quality of care. Let’s work together to support those with dementia in our community and across the UK.

Mental Health

Did you know that studies show that exercising can be as good if not better than giving antidepressants to help depression? Recent studies show that activities such as walking, jogging, and strength training can significantly improve mood and overall mental health, especially in young adults. In light of four tragic suicides of young adults in the Roundshaw area, we at Integrated Neighbourhood Teams are launching a new service focused on supporting mental health through exercise and open conversation. By offering regular group walks, simple workouts, and a safe space to talk, we hope to foster a sense of community and belonging for those who might be struggling.

Healthy Living

In collaboration with the London Borough of Sutton, the NHS, and the Voluntary Sector we are focusing on healthy living to bring you the best services to help you with your weight and well being.
